§ 30-201. Application.

Any person desiring to operate a food establishment shall make written application to the regulatory authority for a permit, license or certificate on forms provided by the city. Such application shall include the name and address of each applicant, the location and type of the proposed food establishment and the signature of each applicant. Temporary and mobile food service establishments shall apply for a permit at least seven days prior to the date the food service is to commence operation, provided that this seven-day advance application may be waived by the regulatory authority if the applicant shows just cause for not filing the application seven days in advance.
